FOR VISION SCREENERS

The letters and forms on this page are the documents provided in the Vision Screening Requirements and Guidelines for Preschool and School-Aged Children 2017 published by the Ohio Department of Health. They can be downloaded, printed, and some can be edited with your school name, logo and student information.

FOR TEACHERS

Prevent Blindness believes that educators are on the front line of making sure that kids know how to protect their precious gift of sight.  We have curricula for pre-k to high school to help you plan ​educational activities for the kids in your class.
